To update or add questions to the Survey form, follow these steps: 

Login >> Survey >> Click on the "Manage" button on the particular Survey >> Click on "Setup" in the left panel >> Survey Form                          

On this page you will be able to add fields to the form. 

  1. Click on the Field types available to include it.
  2. Click on the action button next to the particular field and provide the Label / question and click on the checkbox to make a field mandatory for respondents to answer.      
  3. You can drag and drop the questions to set the order of the form.
